Family Physicians

Family Physicians demonstrate competence in the patient-centred clinical method; they integrate a sensitive, skillful, and appropriate search for disease. They demonstrate an understanding of patients’ experience of illness (particularly their ideas, feelings, and expectations) and of the impact of illness on patients’ lives.

The Connexion Family Health Team is a bilingual family health team offering a comprehensive range of services; the team is made up of 16 physicians, 14 interdisciplinary care providers, and nearly 20 administrative employees.

Physicians at Connexion Family Health Team will refer, as needed, to the many internal experts, i.e. receptionists, nurses, nurse practitioners, pharmacist, dietitian, social workers, psychologist and health educator to help identify and support comprehensive patient care and individualized treatment plans. Respectful, collaborative, evidence-based and patient-centred care are at the very core to the treatment received by our patients.

Health Educator/Program Administrator

The Health Educator/Program Administrator role focuses on health promotion, illness prevention, and integration with other health services in the community.  The Health Educator works with other members of the Family Health Team to ensure that all patients are receiving the most effective health care that is available to them in their community.

Our Health Educator will promote the following:

  • Education and access to all CFHT nursing, mental health, dietitian and pharmacist-based programs and services
  • Recruit patient referrals and management enrolment for all health-based programs
  • Health Awareness Weeks/Months, i.e. Immunizations, Womens’ Health, Black History, 2sLGBTQ+ Pride
  • Patient Experience Surveys
  • Website and social media platforms

Nurse Practitioners

Nurse Practitioners focus on health promotion and disease prevention. They provide family care from birth to end of life. Nurse Practitioners collaborate with all members of the healthcare team to ensure each patient receives personalized, high-quality service.

The Connexion Family Health Team Nurse Practitioners will:

  • Assess, investigate, diagnose, and treat illnesses both acute and chronic
  • Perform periodic health examination
  • Order and interpret a variety of tests
  • Write prescriptions for medications
  • Provide counselling, education and supportive care
  • Make referrals to other healthcare professionals, including community specialists
  • Specialization in contraceptive care including IUD/Nexplanon Implant insertions and sexual health education

Pharmacist

Pharmacists have a critical role in the delivery and support of medication management services.

Examples of services provided by our Pharmacist include:

  • Medication(s) review and medication adherence
  • Diabetes Management
  • Healthy Aging education
  • Review of Hospital Discharge Plans
  • Smoking Cessation (Quit Smoking) Counselling
  • Education and Teaching to our team members

Psychologist

The Connexion Family Health Team Psychologist is trained to assess, diagnose and treat a full range of emotional and behavioural problems.

The Connexion Family Health Team Psychologist provides counselling, approximately 10-12 sessions, for:

  • Depression, anxiety, fears, or stress at home, school or work or within relationships
  • Individual, relationship and/or family conflict(s)
  • Obsessive Compulsive Disorders (OCD), personality disorders and trauma
  • Chronic pain or adjustment to health conditions
  • Assessment for Adult attention-deficit/hyperactivity disorder (ADHD) 

Registered Dietitian

Registered Dietitians provide practical information about nutrition and lifestyle choices to optimize health. They use various assessment methods to determine patients’ needs and provide evidence-based nutrition recommendations for the treatment of acute and chronic conditions. Registered Dietitians educate and empower individuals and families to take responsibility for their health.

The Connexion Family Health Team Registered Dietitian will:

  • Provide tools to help you manage various health conditions, such as, High Cholesterol, Diabetes, Irritable Bowel Syndrome, Inflammatory Bowel Disease, Osteoporosis, Hypertension, Heart Failure, and Food Allergies or Intolerances
  • Counsel and support you to make changes in your eating habits
  • Provide a framework for transitioning your baby to solid foods
  • Offer educational opportunities for weight management and craving urges, through programs such as, Healthy You and Mindful Eating

Registered Nurse

Registered Nurses have a wide scope of practice and lead many of our health initiatives. They work with the Family Physician and other healthcare professionals to plan and implement healthcare strategies and prevention plans that advance the personal health goals of patients. Registered Nurses provide both direct clinical care and education for individuals and their families.

The Connexion Family Health Team Registered Nurses will:

  • Triage emergency calls and direct you to the appropriate level of care
  • Respond to general questions about your health including medications, guiding you through complicated results, etc.
  • Carry out clinical responsibilities, such as immunizations, allergy shots, TB testing, dressing changes, ear syringing, and suture removal/dressing changes
  • Assist with periodic health and well-baby examinations
  • Perform pap and wellness exams, liquid nitrogen treatment, foot care and sexual health counselling

Social Worker

Social Workers help our patients to restore, maintain and enhance social functioning and personal well-being. Connexion Family Health Team social workers offer individual mental health intake calls and provide one-on-one counselling, 10-12 sessions. In addition, they link people to community agencies, organizations and programs, such as Sleep Hygiene, Mindfulness and Cognitive Behavioral Therapy techniques that aid them in meeting their needs.

The Connexion Family Health Team Social Workers provide counselling for:

  • Problems managing everyday personal, family and work life
  • Depression, anxiety, worries and stress
  • Grief and loss of loved ones
  • Coping with chronic illnesses, new diagnosis and health changes
  • PTSD and trauma
