Please find the full list of the programs we offer below. If your family doctor is part of the Connexion Family Health Team (CFHT), you have free access to all of our programs. Programs are also offered to community-based, non-CFHT patients, Ontario residents, if spaces allows.
To sign up, simply click on the program below and fill in your name and phone number or you may contact reception at 613-656-3841. A member of our team will then contact you with more details. If the program is full, you will be added to the waiting list and contacted when a spot becomes available. There is no fee to participate in any of the programs.
Please note that all programs are either offered virtually or take place at the Connexion Family Health Team location, 5303 Canotek Rd, Unit 8,
Free parking is available on-site. We look forward to supporting you and your health in our selection of wonderful programs.

Flu Shot Clinic Dates & Times
To Be Announced: We offer flu shot clinics each year during the months of October and November. Clinic dates will be announced in September/October.
We would like to encourage patients specifically in the age group 65+ to receive their flu vaccine at local participating pharmacies.
Children under the age of 5 can be booked to receive the flu vaccine by having their parent/s /custodian /guardian contact their physician’s office or Connexion Family Health Team at 613.656.3841 to book an appointment with one of our Registered Nurses. Appointments with the Registered Nurse take place at Connexion Family Health Team, 5303 Canotek Road, Unit 8.

Mindfulness Workshop
The Mindfulness Workshop is an informational session aimed to introduce patients to the concept, and benefits, of mindfulness. In addition to providing an introduction to what mindfulness is and how it can be helpful for patients who struggle with stress management, mild anxiety, mild depression and chronic pain, we will practice a number of quick and easy mindfulness techniques that patients will be able to use in their daily lives. This Workshop is designed for those who are completely new to mindfulness and will offer a number of resources for further learning and support if interested

Mindfulness Registration
Healthy Aging
Healthy Aging program addresses common risks associated with aging including malnutrition, social isolation, falls, and potentially inappropriate over-the-counter and prescription medications. Community resources and supports that are available will be shared. The importance of advance directives and powers of attorneys will also be discussed. The Healthy Aging program will allow you to hear from each of our clinicians, at Connexion Family Health Team, including a registered nurse, pharmacist, dietitian, and social work.
Registered Nurse:
This section of the Healthy Aging presentation focuses on falls, the common causes and removing hazards. We use the “Staying Independent” brochure published by the LHIN and the 4 regional Health Units to assess your risks and offer recommendations on staying safe, what to do if you fall and when to seek medical attention. Lastly, but especially important, we will discuss the recommended vaccinations, benefits and scheduling.
Pharmacist:
This section of the Healthy Aging presentation reviews the potential harmful effects of medications, including; falls and fractures, memory problems, hospitalizations, and premature loss of independence.
Dietitian:
This section of the Healthy Aging presentation will review the importance of eating well as we age. Good nutrition can help prevent or manage heart disease, diabetes, osteoporosis, and cognition. It can also help maintain muscle mass, functionality, and balance. Key nutrients will be discussed: role, individual needs and sources of protein, fibre and fluids, calcium and vitamin D. The Mediterranean lifestyle is emphasized: more whole foods, healthy fats, limiting processed foods, and regular physical activity. Label reading and tips for shopping on a budget will be discussed to help you make healthy food choices. Resources provided include lists of local grocery stores that offer delivery and ready-to-eat meal delivery services.
Social Work:
This segment of the Healthy Aging Educational Workshop focuses on mental health and aging, reviewing several aspects including: a discussion of mental wellbeing and mental health, and informing participants that mental health issues can arrive at any stage of life and can be treated. Dimensions of mental wellbeing/healthy ageing are briefly reviewed. Next, we review common sleep issues specific among older adults. The presentation explores isolation, the risks and impacts related to isolation, and identifying potential steps to reconnect and/or build new connections to combat isolation. Many Ottawa based programs that offer opportunities to socialize and connect with others are introduced to participants. We also briefly discuss the risks of connecting with others online, including identifying and avoiding scams.

Contraceptive Care Clinic
The goal of our clinic is to make sure that you have all the information that you need to make an informed decision about what is the right option for you. We do this using a few steps:
1) You will have a chance to ask questions and consider which option(s) you are considering or want to know more about.
2) The Nurse Practitioner will make sure your choice is safe for you and give you a prescription. If required (for example, with an IUD or the implant), they will schedule you for another appointment to start that method.
3) You may or may not need to be seen for a follow up to make sure everything is working properly. This will depend on which option you chose and what the Nurse Practitioner thinks is best for you.

Quit Smoking Program
The Smoking Cessation Program helps educate patients on both the short and long term health effects of smoking cessation. Discussion includes information on the most up to date/recommended medications and nicotine replacement therapies as well as strategies to help make the attempt successful. The sessions can be “information gathering only” if patients are not immediately ready to quit.
Setting a quit date during the counseling session is encouraged but not mandatory.

CBT Workshop
The CBT workshop is a two part workshop reviewing some of the primary strategies used in Cognitive-Behavioral Therapy, also known as CBT, to treat mental health challenges. We will explore how to understand symptoms of depression and low moods, anxiety, and other mental health challenges through the CBT model. We will also explore effective strategies to apply, such as the 5-Part model, behavioral activation and activity scheduling, relaxation strategies, thought records and more.

This workshop is intended for people experiencing insomnia. Topics covered include sleep education, sleep hygiene, relaxation exercises, cognitive-behavioural therapy (CBT) tools for insomnia (sleep diaries, thought records and worry time) and mindfulness. Further resources are suggested, including individual counselling for sleep problems.
