Appointments

To book an appointment with a physician you must call the Queensway Medical Centre at 613-746-6223 (doctors working out of Connexion Family Health Team belong to the Queensway Medical group). Phones are answered from Monday to Thursday 8:00am to 4:00pm, Friday 8:00am to 11:00am.

For appointments with the Nurse Practitioners, Registered Nurses, Social Workers, Psychologist, Registered Dietitian, Physiotherapist and Sport Chiropractor please phone the Connexion Family Health Team at 613-656-3841. Phones are answered from 8:00am to 5:30pm Monday to Thursday and 8:00am to 4:00pm Fridays.

Bloodwork

  • You will be called about any abnormal test results and a follow-up appointment will be booked.
  • The secretary does not have the necessary medical knowledge to discuss results with you.
  • You will not be called about normal results.
  • If you would like to review any test results, please book an appointment.
  • HIV Testing - You can book a follow-up appointment at the time you are provided the requisition for 2 weeks.  You will be called whether these results are normal or abnormal.

Fees

Not all services are covered by OHIP. Fees are associated with the following:

  • Mental Health Workbooks  $15.00
  • Sports Chiropractor and Physiotherapist Initial visits are $90.00
  • Sports Chiropractor and Physiotherapist Follow Up visits are $65.00

Connexion Family Health Team has FREE PARKING directly in front of the building with two handicap spots in front of the entrance.

Prescriptions

All prescription repeats are done by faxing.  If a patient needs a repeat of their prescription, they are to phone their pharmacy and their pharmacy will fax your Family Physician with a hard copy request. Your doctor will then fax the pharmacy with a renewal. Please give 72 hours for turnaround time.

Referrals

Referrals are either faxed to the Specialist or contacted by phone.  Patients will be contacted by either their Family Physician’s office or directly by the Specialist's office when an appointment is made.  If you have not heard about your referral within 1 month, please call your doctor’s office to inquire about the status of the referral.

Test Results

You will be called about any abnormal test results and a follow-up appointment will be booked.

The secretary does not have the necessary medical knowledge to discuss results with you.

You will not be called about normal results.

WSIB

WSIB forms are to be scheduled with your Family Physician.

To book an appointment with a physician from Queensway Medical Centre, please call 613-746-6223. Phones are answered from Monday to Thursday 8:00am to 4:00pm and Friday 8:00am to 11:00am

Bloodwork

  • Usually we will not call you about normal results.
  • You will be advised about any abnormal test results and a follow-up appointment may be booked to review results with your Physician.
  • The secretary does not have the necessary medical knowledge to discuss details of your results with you. If you need more information regarding your results please schedule an appointment with your Physician.
  • If you would like to review any test results, please book an appointment.
  • HIV Testing - Depending on your Family Physician you may have to book a follow-up appointment for 2 weeks after being tested for HIV. Your Family Physician will discuss and review the results with you.

To book an appointment with Dr. Hodgson or Dr. Meehan, you must call us at 613-830-5888. Telephones are answered Monday to Thursday 9:00am to 3:45pm and Friday 9:00am to 11:45am.

**note** closed for lunch 11:30am to 1:30pm Monday to Thursday.

Fees

Call the office to acquire information regarding all non-medical fees that are not covered by the Ministry of Health, Third Party, Private Billings, and Out of Province services, as well as any costs incurred for travel.

Free parking is available at our office location.

The following bus routes are regular hours: 132 and 134; while the 193 is available at xpress route hours.

Prescriptions

All prescription requests are done by fax. If a patient requires a renewal of a prescription, they must contact their pharmacy directly and the pharmacy will fax the request to our office. You will be contacted by the office if the physician requires an appointment in order to renew the prescription. Otherwise, please allow up to 5 working days in order for the request to be processed.

Referrals

You will be contacted by our office when the referral has been scheduled. If you have not heard from our office 1 month after you were informed that a referral was being made for you, please contact us.

Test Results

You will be called regarding any abnormal test results and a follow-up appointment may be necessary. You will be called either by our office staff or by one of the nurses in our Family Health Team.

WSIB

Please inform the receptionist when booking an appointment if this is a WSIB matter. Additional information is required when booking an appointment.

Bloodwork

Results are usually received in 2-7 days. You will be called about abnormal test results and a follow up appointment may be booked. If you would like to review any test results, please book an appointment.

Fees

Chart transfers - $25-$100, depending on the size of the chart. Medical forms/certificates – from $25 and up depending on complexity. Please note all charges require cash cheque or payment.

  • Tuesday-Thursday 7:30am to 3:30pm
  • Friday 7:30am to 2:30pm

Referrals

Your family physician arranges appointments with a specialist by fax or phone. Patients will then be contacted by their family physician office or by the specialist office (depending on specialist booking procedure). If you have been told that you should be hearing about an appointment by a certain length of time and you have not heard by that designated date, then call the family physician office to inquire about the status of the referral request.

WSIB

Please inform the receptionist when booking the appointment if it is a WSIB appointment (ie: If the appointment will be dealing with a work related injury or illness that will be filed with WSIB for a claim)
